Local Excursion by Foot Permission Form

Dear Families,

Over the next 12 months the children may attend local excursions by foot.

Engaging with the local community provides children with the opportunities to contribute to and explore the world around them, learning through real life experiences. 

The area included will be 2km radius each way from Chabad Malvern Creche ( Please click here to see map with common places we may visit)

Chabad Malvern Creche has organised these local excursion for your child to attend. All of the relevant details are provided for your convenience. The service has conducted an Excursion Risk Assessment for each place which is available for your review up request.

For your child to attend, you are required to complete the Authority for Excursion Form.

To complete this form, you must be listed on the child's enrolment record as either their parent/guardian of an Authorised Nominee. 

 Route to known places:

- Malvern Library & Malvern Police station-  Exit 353 Glenferrie Rd to the right, walk down to the pedestrian lights of Glenferrie/High St intersection.

-Glick Malvern &  Coles Malvern - Exit 353 Glenferrie Rd to the right, walk down to the pedestrian lights of Glenferrie/High St intersection and cross over the road and walk down.

- Malvern Public Gardens - Exit 353 Glenferrie Rd to the left, walk down to the pedestrian lights of Glenferrie/Malvern St intersection and cross over the road, walk up Glenferrie rd and turn left into Haverbrack ave, turn right into Spring St, Cross over the pedestrian crossing to the side of the Spring Street School and enter park.

By signing the Authorisation for Excursion Form, I agree to and understand the following:

- My child has my permission to attend the excursion listed. If the excursion is a regular outing, my child has permission to attend for 12 months after the date listed below

- I am listed on the child's Enrolment Form as a Parent/Guardian or Authorised Nominee.

- I have read all of the details provided by the service and understand that I can access the Excursion Risk Assessment at the service.

- The contact details, including all emergency contact details, listed on my child's Enrolment Form are up to date. If not, I have provided the updated contact details above.
