      You will need to FIRST fill out an enrollment form online BY CLICKING HERE:

WHEN  BOOKING IN PLEASE NOTE:
*At the first drop down menu choose Chabad Malvern Vacation Care
*Form will not submit without CRN (for child and primary parent), Hebrew Name (if unkown please type Unknown), immunisation history statement and photo of child,
*Once the booking has been confirmed,  you will receive an email confirmation, this usually takes up to a week. 
* Bookings will close 12th September or if all spaces are filled prior to this. 
* Cancelations after 12th September will still be charged at full fees.

Childcare Subsidy


The Childcare Subsidy is available at our service. To ensure you receive your eligible subsidy, please make sure to include the CRN and Date of Birth for your children and the primary parent when applying online. You will need to ensure your child is registered for Chabad House of Malvern/Toorak Inc- VAC. If this information is not showing on your Mygov account please contact [email protected].

TERMS AND CONDITIONS

  • By clicking the above links and booking your child in you have agreed to our terms and conditions. 
  • A minimum of two days of bookings is required.
  • Once you have received confirmation via email your booking is confirmed. Cancellations after 12th September will still be charged at full fees.
  • Fee's will be charged by the nominated credit card provided or direct debit after your children have attended the program.
  • Your child will not be allowed to attend Chabad Malvern Holiday Program without us obtaining a copy of your child's immunisation status. This is a mandatory requirement by the government.
  • If your child has a medical condition they will not be able to attend Chabad Malvern Holiday without the parent/guardian proving Chabad Malvern with the required information and medication.  
  • If you wish to receive the Childcare Subsidy it is your responsibility to make sure you have fulfilled all government requirement by the onset of the program. If not you risk paying full fees
  • The contact details, including all emergency contact details, listed on my child's Enrolment Form are up-to-date.
  • Once you have received confirmation via email your booking is confirmed. Cancellations after 12th September will still be charged at full fees. However, if cancellation is due to illness you will be credited for next program upon receipt of doctors note. 
  • If your child has a medical condition they will not be able to attend Chabad Malvern Holiday without the parent/guardian providing Chabad Malvern with the required information and medication.  
  • If you wish to receive the Childcare Subsidy it is your responsibility to make sure you have fulfilled all government requirement by the onset of the program. If not you risk paying full fees
  • The contact details, including all emergency contact details, listed on my child's Enrolment Form are up-to-date.
  • Parents agree to have their child’s temperature taken by a staff member prior to entry to the Service
  • Parents agree to have their child excluded if their child’s temperature is above 38°C
  • Any person (employee, enrolled child, parent, caregiver, visitor or contractor) who is displaying symptoms such as: fever, coughing, sore throat, fatigue and shortness of breath, must seek medical attention and not attend our Service under any circumstance
