Sign Up in 4 Simple Steps:

Step 1

Complete the online Chabad Malvern Child information form. This form manages the legally required information for childcare purposes such as your personal information, information about your child and certain authorisations. It is a mandatory pre-requisite for booking into any of our programs. Please note, this is not a booking form.

You only need to be complete this form once. If you have already completed this form before for Chabad Malvern Hebrew School and or Holiday Program and there has not been any changes to your details then there is no need to complete it again.


Step 2

Email a copy of your child's immunisation status to OR Mail to Chabad Malvern Youth, 353 Glenferrie Rd Armadale 3143.

Step 3

If your child has been diagnosed with any of the following you will need to email or mail us an action plan completed by a doctor (must include a photo of your child):

  • Asthma
  • Anaphylaxis
  • Epilepsy
  • Diabetes
  • Allergies/Intolerances (food, diet, sunscreen, band aids etc).
This only needs to be sent once, unless your child's diagnoses changes.

Step 4

Complete the online Program Booking and payment form. This form manages your child's booking for the upcoming Holiday program.
Bookings will close strictly on Friday 7th December or if all spaces are filled prior to this.
